Sometimes the most significant threat to maintaining an active, enjoyable life is not a sudden medical emergency, but a gradual decline in wellness. Larksfield Place is proactively helping minimize such declines. Our Triage Committee is a group of caring staff members who can be thought of as a neighborhood watch team for wellness.
The committee is comprised of the Directors of Nursing, the Fitness Club, the Dining Room, Community Services, Housekeeping, and Supportive Health Services, and is led by Diana Casebolt-Hayes, Vice President of Independent Living. “Individually, we may not see every community member every day but collectively, this group is able to stay in touch with our entire community,” she says.
The Triage Committee meets weekly to share observations. If ongoing concerns are raised about a particular community member, Diana calls on the individual. One such call came to Jane Young, a Larksfield Place community member since April 2005. “They provided what I needed, when I needed it,” she says.
Diana’s call resulted in connecting Young with Amy Hall in the Fitness Club, who created innovative therapies to specifically benefit Young’s situation. As a result, Young is feeling stronger and well-cared for. “There is a fine line between being nosey and being caring, and everyone here respects that line,” she says.
Casebolt-Hayes always begins her visits with reassurance. “I am here to help them be successful in their current situation,” says Diana. After an assessment, options for Larksfield Place support services are suggested—from meal delivery to personalized therapies through the Fitness Center or a temporary stay at the Health Care Center. The community member and family always choose what is appropriate, unless a problem becomes a threat to their personal safety or the safety of others. “In offering this continuum of care, we build trust with members and families,” says Diana. “We are a community looking after community.”
